Is the program reviewed on an annual/as needed basis?

Has a list of on-site and off-site emergency services been compiled for use?

Are these services/agencies notified in the event of a crisis?

Is the person requesting assistance required to initiate evacuation of area?

Do the employee evacuation/relocation procedures use this schedule:

 

  • The complete evacuation of all employees from the facility?
 

  • The dispersal of all employees from department/surrounding department?
 

  • Charge those not injured to assist/help guide those in a crisis to nearest exit?
 

  • Charge the uninjured to assist the injured in a crisis to the assembly point?
 

  • Charge company escorts to aid visitors during any emergency?

Are the types of emergencies that would disrupt the company identified?

Do the contingencies include medical emergency situations?

Does the contingency for containable and non-containable fires include:

 

  • Fire extinguisher training for all employees per 29CFR 1910.38?

Has a company [relocation/roll call/missing person search] point been set?

Is emergency management consulted before workers can return to work?

During an emergency, do employees know the type of evacuation needed?

Is there an employee alarm system in place for warning of:

 

  • A necessary emergency action/reaction time for safe employee escape?

Is the alarm perceptible to affected workers above ambient noise/light levels?

Are emergency numbers placed near the preferred means of reporting crises.

Are procedures for sounding workplace emergency alarms established?

Are all alarm systems maintained in operating condition unless under repair?

Are all alarm systems tested for reliable and adequacy every two months?

Have all types of warning systems been determined for use by the employer?
